Our Services
At Your Journey Home Care Solutions, we tailor our care to meet your needs—right at home. As a Community First Services and Supports (CFSS) provider, we offer a wide range of PCA/CFSS services designed to support our clients in their daily activities and improve their quality of life. Our services include:
- 🏠ADLs: Assistance with bathing, dressing, eating, grooming, and personal hygiene, mobility support and transfer assistance, helping clients move safely within their home and outside, routine physical activities and wellness monitoring,
- 💊Medication Reminders: Timely non-clinical reminders to stay on schedule with prescribed medications
- 🛍️IADLs Household Tasks: Light housekeeping, laundry, and other chores to maintain a clean and safe living environment and grocery shopping and meal preparation: preparing nutritious and delicious meals tailored to dietary needs.
- 🤝Companionship: Providing social interaction and emotional support to combat loneliness and isolation.
- 🚗Transportation: Helping clients travel to appointments, errands, and social activities safely.
